Statement of Engagment and Confidentiality
Formal document to be signed by mentees and both their mentors.
Must be submitted no later than October 13, 2025, to team@cbitn.ca.

Meeting Log
Mentees are asked to track all meetings with their mentors using the Meeting Log template.
These should be saved and submitted at the end of the funding year.

CBITN asks that its funded mentees acknowledge CIHR and the CBITN platform in all relevant publications, presentations, and products resulting from any research associated with their CBITN funding.

Internship Evaluation (Supervisor)
To be completed by the internship supervisor once the internship is completed.

Internship Evaluation (Trainee)
Guidelines:
Should be between 1,500 and 2,000 words.
Should be submitted to CBITN at team@cbitn.ca.
The report should briefly answer and elaborate on the following questions:
1. Introduction: Details of the Host Organization/internship placement
2. What were your duties and responsibilities as part of your role as Intern? How did you fulfill them?
3. What areas/skills have you learned/developed throughout the internship? Provide concrete examples.
4. How can you apply the areas/skills you’ve learned/developed to your professional journey more broadly?
5. Conclusion: Was your internship experience enjoyable and would you recommend this Host Organization to others for future internship opportunities?
